SAP – Substance Use Disorder Nurse, LVN, LPT
POSITION SUMMARY:
The Substance Use Disorder (SUD) Registered Nurse/Licensed Vocational Nurse/ Licensed Psychiatric Tech will properly and precisely prepare daily or weekly medications to our clients. The position will report to the Senior Director and Program Director. The SUD Nurse is responsible for providing care by managing physical needs, preventing illness, and treating health conditions by observing and monitoring participants. The SUD Nurse is also responsible for recording any relevant information to aid in the treatment decision making process

RESPONSIBILITIES
- Must be able to perform health physicals and administer Tuberculosis
- Must be able to complete and understand Incidental Medical Services (IMS) and Medication Assisted Treatment (MAT).
- Represent GLOM in areas of contact with internal people, external contract agencies and community contacts.
- Establish and maintain a therapeutic relationship with clients being
- Assist/Perform all areas of basic nursing
- Count and store medication in correlation with DHCS regulations. Document all medications on Centrally Stored and Destruction Records, create Medication Administration Records as per each doctor order, count and control all medication brought into and out of the facility.
- Maintain a clean and debris free workspace (medication room).
- Maintain a professional working relationship with contract agency staff, doctors, clinicians and case
- Transcribes physician orders accurately and in a timely manner and provide staff with full
- Monitor self-administration of medication to clients and provide education pertaining to said prescribed medication.
- Follow and uphold all medication management policies and procedures.
- Count all medications at least once per shift and audit the Medication Administration Record for detail and accuracy.
- Provide incoming shift staff with a detailed pass down relating to all medication issues.
- Document and notify the prescriber of any changes in behavior or condition relating to
- Actively participate in multidisciplinary team meetings and treatment planning meetings as related to clients and
- Attending scheduled contract agency meetings as a GLOM SAP
- Demonstrates knowledge of crisis management and intervention.
QUALIFICATIONS
- Current RN or LVN licensure, in good standing in the State of
- Five (5) years of experience in behavioral health, understanding of psychiatric illnesses and treatments, knowledge of psychotropic
- Previous experience with the population to be served and/or work in a similar program setting
- Knowledge of Substance Use Disorder, treatment, and detox/addiction medications related to Substance
- Ability to read and transcribe doctors’ orders accurately.
- Ability to provide nursing care and first aid as
- Ability to administer oral and injectable
- Ability to perform vitals and FSBS
- Demonstrate the ability to act
- Demonstrate the ability to act in an appropriate manner in a crisis.
- PPD test for tuberculosis and satisfactory results.
- Free from State
- Valid driver’s
- CPR/First Aid/BLS certification.
PHYSICAL REQUIREMENTS
- The ability to climb stairs multiple times a day and be able to lift and/or carry 25 pounds or
- Ability to sit, rise, kneel, squat, twist, pull bend multiple times a
- Ability to handle hazardous waste (Bloodborne pathogens, hospital waste, chemical & infectious) and sharp instruments and
- Ability to walk with items in hand, grasp and reach as
- Ability to provide as needed bedside care and first aid, as
- Ability to perform with repetitive maneuvers and manual
